A variety of legal forms and packets are available from our three libraries. To request a form, follow the purchase instructions for each library. Pick up forms at one of our libraries, or order forms through the mail.

Wisconsin State Law Library
These fill-in-the-blank forms may be purchased from the Wisconsin State Law Library. In addition, librarians can help you locate sample forms contained in various legal practice manuals. Library staff do not provide help with filling out forms. Contact a librarian with questions or for help with a custom order.
A non-refundable payment processing fee is charged for each online payment. A $3.00 shipping fee is charged for mailed orders, plus the cost of the forms. These forms cannot be emailed or faxed.

Prepayment is required for forms and custom orders. There are several ways to pay. A 5.5% sales tax is added when applicable. Tax exempt and out-of-state customers do not pay sales tax. For custom orders and payments, first contact a law librarian with your request to verify the amount due.

Visa or MasterCard credit card payments are accepted online, by phone (800-322-9755), or in person. A non-refundable payment processing fee is charged for each online payment.
Check, Cash or Money Order
Do not send cash in the mail. Be sure to include a $3 shipping fee with your payment. Mail your payment, a note stating what you've requested, and your mailing address to:

Dane County Law Library

To purchase these forms, please stop in and pay in person or mail payment for forms plus a $3.00 handling fee with your request to DCLL at 215 South Hamilton St., Room L1007, Madison, WI 53703. We accept credit cards, money orders, or cash.
In addition to these forms and the forms online, DCLL has many sample forms for legal or court actions for which there is no official form. Library staff can help you locate a form for your needs, but cannot help you fill it out.
